Polymyalgia Rheumatica, an Age-Related Rheumatic Disease

چکیده
Polymyalgia rheumatica (PMR) is an age-related chronic inflammatory disease with rheumatic features at the fore. In addition to high-grade systemic inflammation, it characterized by typical “polymyalgic” musculoskeletal symptoms, including diffuse and severe pain prolonged morning stiffness of shoulder girdle, pelvic neck. PMR a member so-called giant cell arteritis complex; however, in spite marked inflammation PMR, local vasculitis process aborts. The pathological background synovitis, predominant extra-articular synovial structures. Synovitis mild, transient, non-erosive. Distal symptoms are also observed but more variable less recognizable than proximal polymyalgic syndrome. often overlaps elderly-onset seronegative arthritides, rheumatoid arthritis, late-onset spondylarthritis, RS3PE1 Although glucocorticoids cornerstone therapy, considerable hope attached tocilizumab, IL-6 receptor inhibitor.

Peripheral arterial disease in polymyalgia rheumatica
Patients with polymyalgia rheumatica have been shown to have an increased risk of peripheral arterial disease on longitudinal follow-up. Possible explanations for this include premature atherosclerosis related to chronic inflammation, as with other inflammatory rheumatological conditions. Alternatively, polymyalgia rheumatica can be associated with vasculitis, even in the absence of clinical gi...
متن کاملA disease activity score for polymyalgia rheumatica.
OBJECTIVE To develop a composite score for measurement of disease activity in polymyalgia rheumatica (PMR) and assess its internal and external validity. METHODS A PMR activity score (AS) was designed and assessed for internal and external validity in two patient cohorts: 57 international patients evaluated primarily for development of the PMR-AS at baseline, weeks 4 and 24; and for validatio...
متن کاملUpdate on polymyalgia rheumatica.
Polymyalgia rheumatica is an inflammatory disease of unknown etiology affecting individuals aged fifty years and older, mainly of Caucasian ethnicity. Polymyalgia rheumatica is associated with giant cell arteritis more frequently than expected by chance alone. In both conditions, females are affected two to three times more often than males. The clinical hallmark manifestations of polymyalgia r...
متن کاملAn evaluation of criteria for polymyalgia rheumatica.
There has been little basis on which to standardise a diagnosis of polymyalgia rheumatica (PMR), and so 11 rheumatology units in the south and west of Great Britain have collaborated in a study to evaluate possible criteria. Symptoms and laboratory findings claimed to be of diagnostic value in PMR were included in an analysis of the features of 236 patients considered to have unequivocal PMR an...
متن کاملPolymyositis, not polymyalgia rheumatica.
The distinction between polymyalgia rheumatica and polymyositis is important for treatment and prognosis. Four elderly patients were diagnosed and treated for polymyalgia: raised creatine kinase led to muscle biopsy and a diagnosis of polymyositis.
